About the Role

The Care Manager will link, advocate, and support youth and families to improve their overall health and wellness. This role involves completing various assessments, facilitating team meetings, and coordinating services with multiple stakeholders.

Responsibilities

  • Provide overall support to youth to ensure they receive necessary services for health and wellness goals.
  • Demonstrate understanding of the four dimensions of safety and identify gaps.
  • Demonstrate and model sensitivity to the cultural background of children, families, and co-workers.
  • Ensure job-related activities comply with Glove House policies, Department of Health, Care Management Agencies, State and Federal regulations, and professional standards.
  • Perform care management tasks as defined by the Department of Health and contracted Care Management Agencies (e.g., CHHUNY), including assessments, goal plans, and safety plans.
  • Link, advocate, and support youth and families by identifying strengths and barriers, providing referrals, and offering interventions for psychosocial support, medical, dental, behavioral health, educational, employment, and transportation needs.
  • Participate as a team member of the Health Homes Department and Finger Lakes Regional Office, providing support to other teams as needed.
  • Coordinate services with other professionals and paraprofessionals, and liaise with outside social service agencies and organizations.
  • Provide comprehensive, client-centered, trauma-informed, collaborative care planning for youth and parents/guardians to integrate medical, behavioral health, and social health services.
  • Utilize effective communication strategies with peers, medical staff, addiction and mental health providers, and community agencies using electronic devices, Telehealth, and other interactive technology.
  • Help improve, measure, monitor, and sustain quality outcomes focusing on clinical indicators, patient satisfaction, and plan adherence.
  • Participate in interdisciplinary team meetings and conduct regular face-to-face contact with youth and families.
  • May monitor interns and/or volunteers.
  • Develop and maintain records and program documentation, including assessments, care plans, visitation plans, progress notes, and summaries, according to contract and Glove House standards.
  • Generate and maintain necessary reports and paperwork (e.g., Quality Assurance and program reports).
  • Ensure all documentation is completed within 48 business hours of contact.
  • Ensure program staff are updated on the concerns and needs of the assigned caseload.

Requirements

  • Bachelor's degree required.
  • CHUNNY certification preferred.
  • Minimum 2+ years' experience working with children and families in a residential, group, or counseling child welfare capacity.
