About the Role

The Day Habilitation Team Leader is responsible for ensuring the delivery of quality services to enrolled individuals, fostering independence through Day Habilitation Action Plans, and overseeing daily tasks and routines. This role involves participation in an Interdisciplinary Treatment Team and requires maintaining a professional and responsible work environment.

Responsibilities

  • Complete daily communication log assignment sheets by assigning individuals and responsibilities to DSPs each morning.
  • Ensure all assigned tasks such as attendance, appointments, and outings are completed.
  • Ensure all billing and outcomes sheets are completed in a timely manner for each individual.
  • Meet weekly with the Day Hab Program Coordinator to report issues, review the past week, and plan for the following week.
  • Review all progress notes in the system (ECR) daily and ensure monthly summaries are completed and up to date.
  • Review and sign off on all outing forms.
  • Attend life plan meetings with the program coordinator to ensure updates are accurate.
  • Ensure all volunteer activities are completed weekly.
  • Contact IRA managers, family members, and care managers as needed.
  • Communicate and coordinate with nursing staff to ensure individual’s medical needs are met.
  • Prepare documentation and ensure monthly summaries are completed by the 15th of each month.
  • Oversee the cleanliness and maintenance of the program facility daily and assign staff cleaning responsibilities.
  • Submit work orders for repairs as needed.
  • Maintain professional dress and attitude as a role model.
  • Ensure transportation for individuals to and from program, activities, and other purposes occurs as scheduled.
  • Maintain agency vehicles to ensure safety.
  • Submit all gas receipts to Program Coordinator on a weekly basis.
  • Ensure fire safety needs are met, including planning and executing quarterly fire drills.
  • Maintain individual’s records on the OPWDD Choices systems and the electronic record ECR.
  • Ensure the safety and wellbeing of all individuals at all times.
  • Attend mandatory training and/or meetings as scheduled.
  • Maintain up-to-date first aid and CPR certifications.
  • Perform other duties as assigned by the supervisor.

  • High School diploma or GED.
  • Current NY State driver’s license.
  • Experience working with community human service providers and working with Developmentally Disabled population.
  • Must be bilingual.
  • Basic cooking skills.
  • Ability to follow oral and written directions.
  • Ability to maintain confidentiality and to follow all Ibero Policy and Procedure.
  • A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted.
  • Occasional standing, squatting, lifting of up to approximately 10 lbs.
