About the Role

Family Service League is seeking a part-time Bilingual Case Manager for its Alternatives for Youth (AFY) Program. The program aims to prevent youth referral to the Suffolk County PINS Probation System by providing care coordination, services, and education to families, emphasizing positive change. Bilingual proficiency in Spanish is required.

Responsibilities

  • Provide linkage, coordination, advocacy, and mentoring.
  • Engage families in conflict management and skill building.
  • Provide direct services, including anger management and parenting skills to youth and parents to avoid involvement with the juvenile justice system.
  • Conduct assessments and maintain electronic health records of all services provided.
  • Conduct home visits throughout Suffolk County.
  • Perform other duties as assigned.

Requirements

  • Bachelor's degree, or Associate’s degree with one year of human service experience, or High School Diploma with three years of human service experience.
  • At least one year of experience working with community resources.
  • Excellent interpersonal, organizational, and problem-solving skills.
  • Strong communication skills, verbal and written.
  • Bilingual and Spanish communication skills.
  • Ability to work as part of a team.
  • Proficient computer skills, including Microsoft Office.
  • Valid and clean NYS Driver's license.
  • Travel required.
  • Ability to sit for long periods and travel throughout Suffolk County to conduct home visits.
