Responsibilities
- Actively engage clients through in-person contact, assess their needs, and provide comprehensive care coordination.
- Identify eligible clients and educate providers on eligibility criteria.
- Collaborate with the lead health home to determine client eligibility.
- Develop patient-centered plans of care and prepare crisis intervention plans.
- Ensure the delivery of all necessary services by coordinating with service providers and health plans.
- Advocate for services and assist with scheduling medical appointments.
- Provide support for transitional care following admissions and discharges.
- Offer patient and family support and connect them to community resources.
- Ensure timely and thorough documentation as required by the Department of Health.
Requirements
- Bachelor's degree in Nursing or Human Services related field
- Minimum of 3 years of experience in care coordination or equivalent combination of education and experience
- Ability to work with people of diverse backgrounds
- Thorough working knowledge of local and regional public and private resources
- Familiarity with Social Security, Department of Social Service Adult Protective Unit, Income Assistance Unit, and other relevant agencies
- Basic computer skills
- Creative problem-solving skills
- Negotiation skills
- Ability to navigate and utilize multiple Electronic Medical Records (EMRs)
- Valid N.Y.S. Driver's License
- N.Y.S. Child Abuse Registry clearance
- Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals
