About the Role

Lenox Hill Neighborhood House seeks a Housing Placement Coordinator to implement a holistic mental health model, assisting clients in achieving long-term stability and securing permanent housing. This role requires a focus on community-based human services with a social justice lens, supporting clients in optimizing wellness and achieving housing placements.

Responsibilities

Manage the housing process for all clients, including identifying housing options, recommending best fits, and ensuring successful application approvals for various housing programs and vouchers.
Provide direct client services, including assessments, case management, escorts, housing support, educational groups, follow-ups, and referrals.
Maintain an active caseload, completing needs assessments and biweekly Individualized Living Plans (ILPs), and providing case management and housing support.
Coordinate the entire housing process from intake to placement, including application completion, voucher eligibility, and housing interview participation.
Assist with treatment planning, case conferences, and staff meetings, collaborating with on-site psychiatric and medical providers.
Help clients achieve stability, obtain housing, and develop skills for permanent housing retention.
Manage client records, housing packages, data collection, and project completion in a timely manner.
Liaise with governmental partners, landlords, and housing providers to track and implement successful housing outcomes.
Collaborate with internal staff and external partners to maximize support and program offerings.
Complete all other assigned responsibilities and duties.

Requirements

Minimum of three years' experience working directly with clients in fields such as homelessness, chronic mental illness, substance abuse, trauma, and/or residential services.
Excellent oral and written communication and organizational skills.
Excellent emotional intelligence, leadership, engagement, and interpersonal skills.
Ability to effectively collaborate with key stakeholders and community providers.
Flexibility and availability to support program operations as needed, given the 24/7 operating schedule and client needs.
Must undergo a pre-employment background check.

Spanish fluency is ideal.

The Women’s Mental Health Shelter is a 24-hour program addressing the critical issues facing 80 homeless women, age 45 and over, with histories of mental illness.
The shelter provides a comprehensive continuum of care including social work, medical, nursing, psychiatric care, peer support, housing placement, benefits assistance, arts, recreation, and nutrition programs.
The multi-disciplinary team helps clients transition from homelessness to permanent housing.
