About the Role
Conifer Park Outpatient Clinic in Troy, NY is seeking a Nurse Practitioner to provide comprehensive medical care, including diagnosis, treatment, and patient management for substance use disorders.
Responsibilities
- Perform comprehensive medical history and examination, and record initial diagnosis.
- Administer or order diagnostic tests and interpret results.
- Assess appropriateness of medication-assisted treatments for substance use disorders.
- Provide induction, monitoring, titrating, maintenance, and tapering of medications as clinically appropriate.
- Evaluate and treat medical emergencies.
- Refer unstable patients to appropriate medical facilities.
- Maintain training in naloxone use and emergency first aid response techniques.
- Instruct and counsel patients regarding compliance with therapeutic regimens.
- Perform pre-admission clearance and interviews.
- Develop and implement patient management plans, medication recommendations, and assist with continuity of care.
- Perform and document follow-up visits, assessing and managing medication effects and side effects.
- Participate in case assignments, conferences, treatment team meetings, and educational presentations.
- Provide medical on-call coverage as assigned.
- Participate in peer review and performance improvement initiatives.
- Prepare and educate patients for discharge medications and instructions.
Requirements
- Post Graduate Degree – Masters in Nursing
- Nursing License to practice in State of New York
- National Certification as Nurse Practitioner (e.g., through American Nurses Credentialing Center, National Certification Center, or American Academy of Nurse Practitioners Certification Program)
- CPR Certification
- Unrestricted DEA Licensure
- DATA 2000 waiver
- Active Medicaid Number (Highly Preferred)
- Background checks, pre-employment & drug screenings required
