About the Role
Cayuga Counseling Services, Inc. is seeking an Adult Intensive Case Manager to provide intensive services to adult clients in the Cayuga County Jail, assisting with their transition back into the community. The role focuses on restoring, rehabilitating, and supporting individuals to become active and productive members of their community with minimal ongoing professional intervention. This includes helping clients acquire and improve skills necessary for successful community living.
Responsibilities
- Provides Jail based in-reach services.
- Receives referrals and initiates contact with identified clients pre-release.
- Completes ongoing risk/needs assessments, including COMPAS and Denver Acuity Scale.
- Develops and updates transitions plans collaboratively with clients based on assessments and changing needs.
- Monitors transitions plans to ensure needs are met and goals are adhered to.
- Coordinates meetings regarding care plan appropriateness with clients, natural supports, and providers.
- Ensures necessary supports are in place for transitions plan execution.
- Establishes and maintains necessary linkages, advocating for additional services as needed.
- Collaborates with service providers to establish team communication and crisis plans.
- Participates in the Multi-Disciplinary Reentry Case Management and Coordination Team.
- Provides rehabilitative services such as social and interpersonal skills, daily living skills, and community integration.
- Offers post-release support, transportation, and assistance to appointments.
- Delivers services in various community locations where clients live, work, attend school, engage in services, and socialize.
- Provides services in a trauma-informed, culturally, and linguistically competent manner.
- Completes all documentation in a timely manner.
- Submits timesheets via the payroll system in accordance with Agency regulations.
Requirements
- A minimum of two years' experience working in the human services field.
- Must successfully pass all required background checks.
- Must have a valid NYS driver's license.
- Must have reliable transportation.
- Must possess excellent verbal and written communication skills.
