About the Role

Assist in the coordination of the residential program for individuals we support, emphasizing independence, integration, individualization, and productivity based on personal choice. Incorporate a person-centered approach into training and proactively provide, arrange, supervise, and document services to foster independence.

Responsibilities

  • Assist individuals we support in purchasing household equipment and managing personal funds, purchases, and disbursements.
  • Help individuals we support acclimate to their living environment.
  • Assist individuals we support with proper nutrition, meal planning, preparation, and grocery shopping, providing training as needed.
  • Guide individuals we support in maintaining the cleanliness of their residence, performing domestic tasks, and upkeep of their rooms and property.
  • Ensure the physical plant is maintained in a safe and sanitary manner.
  • Provide guidance to individuals we support regarding leisure activities and assist with transportation arrangements.
  • Conduct and document fire drills as required by regulation and policy.
  • Communicate the needs of individuals supported and household supplies to the Residence Manager.
  • Participate in staff meetings and discussions concerning individual and household needs and the CR/IRA program.
  • Actively participate during crisis intervention.
  • Implement and follow through on ADL, interpersonal skill training, and behavioral support programs as directed and outlined in support plans.
  • Document program implementation on the same day, ensuring accuracy and reflecting individual progress.
  • Administer medications if designated, after completing the required course and maintaining certification.
  • Attend and demonstrate proficiency in all required in-service training.
  • Report any illness, accident, or changes in an individual's medical status to the on-call nurse or administrator, following appropriate procedures.
  • Complete Nursing Notes daily to reflect monitoring of resident health status.
  • Write incident reports (GERs) as defined by Incident Review Procedures.
  • Ensure the needs of individuals we support are met, considering integration, individualization, independence, and productivity, while prioritizing safety and welfare.
  • Promote the five governing principles to improve residents' lives.
  • Communicate all matters concerning the individual's support program and physical plant to the Residence Manager or Assistant Residence Manager via daily log or direct contact.
  • Accept temporary or permanent transfers within the department as necessary to meet the needs of individuals served.
  • Adhere to policies and procedures outlined in the Functional Plan for the assigned work site.
  • Document service plan documentation per regulations and training, including programming, nursing services, clinical services, and self-care.
  • Monitor individuals we support during sleeping hours and attend to their needs.
  • Perform cleaning duties daily as per the cleaning schedule and any other assigned duties.
  • Complete all cleaning logs and physical plant checklists.
  • Act as designated in charge in the absence of the Residence Manager if so designated.
  • Follow correct procedures for the use of personal funds, ensuring proper purchasing and documentation.

Requirements

  • High School Diploma or equivalency preferred.
  • Experience working with persons with a developmental disability preferred.
  • Valid/acceptable NYS driver’s license.
  • Authorized to work in the United States.
  • Must be progressive and proactive.
  • Must support the goals, objectives, and philosophy of the agency.
  • Must support the Agency’s values by working with measurable signs of PRIDE.
  • All staff shall be cleared by the transportation department to drive an agency vehicle and maintain their driving certification.
  • If hired as non-driver or certification is lost, there will be a reduction in pay.
  • Must successfully complete the OPWDD approved medication course if required to administer medications.
  • Must achieve 80% competency on in-service training exams.
