About the Role

The Care Manager oversees and coordinates access to services for individuals with intellectual and developmental disabilities, supporting them to live meaningful, self-directed lives. This role involves coordinating services, advocating for members, and supporting person-centered Life Plans to drive real impact.

Responsibilities

  • Develop and manage individualized Life Plans based on comprehensive assessments.
  • Coordinate medical, behavioral health, and community-based services.
  • Advocate for members to access services, benefits, and supports.
  • Lead interdisciplinary team meetings and collaborate with providers.
  • Monitor progress, address gaps, and ensure high-quality care delivery.
  • Maintain accurate documentation and compliance with regulatory standards.
  • Support transitions across care settings and life stages.
  • Deliver person-centered care management services in compliance with regulatory standards and agency policies.
  • Complete comprehensive assessment/reassessment processes.
  • Identify service gaps and make appropriate referrals.
  • Develop strategies to address conflicts in the person-centered planning process.
  • Ensure all billing critical documentation is present and valid.
  • Maintain member eligibility for care management and OPWDD eligibility.
  • Enroll in the Home and Community Based (HCBS) waiver.
  • Identify and access benefits and entitlements (Medicaid, Social Security, SNAP, etc.).
  • Ensure existing benefits and entitlements are maintained.
  • Maintain current and accurate information sharing consent.
  • Coordinate and provide access to high-quality healthcare services, including medical, behavioral health, and specialized services.
  • Provide regular communication, monitoring, and follow-up on critical healthcare needs.
  • Identify, coordinate, and provide access to preventative and health promotion services.
  • Coordinate transitional care, including discharge planning and transfers.
  • Foster self-determination and community inclusion through linkage and referral to community resources.
  • Utilize health information technology, including electronic health records and telehealth services.
  • Maintain a thorough and accurate electronic health record for all assigned members.
  • Support member self-advocacy and provide advocacy on behalf of members.
  • Attend department/team meetings, trainings, and supervisions.
  • Complete all required trainings within specified timeframes.
  • Travel throughout the designated service area to meet with members, providers, and interdisciplinary team members.
  • Accompany members to necessary appointments as indicated.
  • Identify and follow all incident reporting guidelines and procedures.
  • Adhere to all policies and standard operating procedures for comprehensive care management.
  • Comply with all standards of conduct, including internal regulations, OPWDD, DOH, and the Justice Center.
  • Maintain confidentiality in accordance with HIPAA and privacy practices.
  • Perform other duties as assigned.

Requirements

  • Bachelor’s degree + 2 years of relevant experience, OR Master’s degree + 1 year of relevant experience, OR RN license + 2 years of relevant experience.
  • Experience in human services, I/DD, mental health, or a related field is preferred.
  • Strong communication, problem-solving, and organizational skills.
  • Ability to work independently in a fast-paced, field-based role.
  • Commitment to person-centered, strength-based care.
  • Must be able to meet members in their homes or other community locations.
  • Travel to off-site locations is required.
  • Must be comfortable driving.
  • Must reside in New York State, or a contiguous state (CT, NJ, PA, VT), and within 100 miles of the assigned office Hub.
  • Must possess proof of a valid Driver’s License from New York or a contiguous state.
  • Must possess a dependable vehicle for frequent travel.
  • Proof of valid auto insurance for standard personal vehicles will be required at the time of hire.
  • Excellent interpersonal, public speaking, and written communication skills.
  • Ability to work autonomously.
  • Demonstrate professionalism, respect, and ability to work in a team environment.
  • Absolute sense of integrity and personal commitment to serving people with I/DD and their families.
  • Employee must have access to a password-secured WiFi connection and/or ACA/NY Mobile Hotspot.
  • Employee must be able to secure any printed materials containing individual or employee personal and/or health information.
  • Employee must be able to conduct phone and video calls in a private area where the conversation cannot be overheard and their screen is not visible to others.
  • Employee must be able to conduct work in a location where personal and health information on a monitor is not visible to others.
