About the Role
Visiting Nurse Service Westchester is seeking a Physical Therapist to provide direct patient care, develop treatment plans, and educate patients and families in the home setting. This role involves continuous re-evaluation of patient conditions and collaboration with healthcare teams.
Responsibilities
- Assess patients for functional limitations and develop therapy treatment plans with specific goals in collaboration with the patient, family, nurse, physician, and other health team members.
- Provide direct care to treat medical/surgical conditions using accepted physical therapy modalities.
- Instruct and supervise patients, families, and home health aides in home exercise programs and other aspects of patient care.
- Document patient response to instruction.
- Continuously re-evaluate patient conditions and modify treatment plans as appropriate.
- Identify and report significant clinical information to the physician, clinical manager, primary nurse, and other disciplines.
- Document all clinical evaluations, treatments, and patient responses to therapy.
Requirements
- NYS professional license and current registration as a Physical Therapist.
- One (1) year of full-time experience in the direct provision of physical therapy services.
- Ability to assess and respond to the needs of patients and families in the home setting.
- Ability to utilize industry technology such as a tablet when in the field with patients.
- Ability to drive with an automobile in good working condition and available for use in the field.
Skills
- Home care experience preferred
- Experience with Oasis assessments is preferred
