About the Role

The Geriatric Wellness Coordinator provides on-site wellness services to meet the behavioral health and social needs of residents in Supportive Housing Programs for adults and seniors. The role focuses on supporting residents in maintaining permanent affordable housing and aging in place with dignity, while upholding agency mission and best practices.

Responsibilities

  • Provide supportive mental health counseling to tenants.
  • Meet with assigned residents regularly to discuss and document progress.
  • Provide groups for seniors on benefits, entitlements, wellness, and socialization.
  • Facilitate psychoeducational workshops.
  • Respond to referrals in a timely and professional manner and follow up on referrals.
  • Conduct and prepare intake assessments, case logs, case notes, and service plans.
  • Establish and maintain contacts with families, social service support agencies, and health facilities.
  • Assess residents' needs and develop and implement support plans.
  • Assist residents in developing and achieving meaningful service plan goals.
  • Assist residents with arrears in obtaining OSD or financial management assistance.
  • Develop wellness goals and activities with resident and staff input.
  • Implement and promote wellness activities.
  • Ensure consistent development of appropriate wellness, educational, and social programming.
  • Coordinate care for targeted critical/high need resident cases.
  • Provide Crisis Intervention and crisis follow-up services.
  • Work in a team approach with property management and maintenance staff to identify and respond to residents in need.
  • Model positive advocacy and help residents self-advocate with community service providers.
  • Identify service gaps that impede service delivery and efficiency.
  • Provide ongoing, active outreach and creative engagement of residents.
  • Maintain contact and follow-up with service providers for residents.
  • Adhere to and enforce Agency policies regarding Confidentiality, Corporate Compliance, and HIPAA.
  • Accompany residents to face-to-face meetings as requested.
  • Attend staff meetings, agency meetings, workshops, classes, and in-service trainings.
  • Perform other duties as assigned.

Requirements

  • Bachelor’s degree in human services, Sociology, Psychology, Social Work or related field required.
  • Master’s degree in psychology, Mental Health Counseling, Social Work preferred.
  • One year experience working in housing, mental health, senior and/or elderly populations or related fields.
  • Excellent organizational, verbal, & written communication skills.
  • Computer literate, including experience with all Microsoft Office software packages, local area networks, and the Internet.
  • Ability to work independently as well as part of a team.
  • Bi-Lingual Chinese/Korean/Spanish/English Preferred.
  • Regularly required to talk, hear, walk, stand, & sit.
  • Able to climb stairs if necessary.
  • Able to operate a computer keyboard, mouse & office equipment.
  • Ability to read printed materials and computer screens.
  • Ability to work flexible hours (evenings/weekends) as assigned.
  • Able to travel throughout the five boroughs as needed.
  • Ability to lift up to 10lbs.
