About the Role

AMANI is seeking a Nurse Practitioner to provide consultation and support to Black youth and families navigating mental health and substance use concerns. The role involves working within an interprofessional team, providing comprehensive care, and collaborating with various internal and external partners.

Responsibilities

Provide consultation to other professionals working with the target population.
Consult and collaborate with the Virtual Care Team (VCT) to support client navigation to appropriate care providers and resources.
Monitor, triage, and process referrals.
Coordinate and facilitate clinical/educational group sessions.
Document participant progress.
Provide medication management, housing and financial support, and psychosocial support.
Conduct health assessments.
Provide follow-up and treatment.
Offer expert consultation to clients and the interprofessional team.
Plan, implement, and evaluate direct care.
Provide clinical leadership for patient and family care.
Communicate diagnoses, prescribe medications, and order diagnostic tests autonomously.
Lead and manage crisis responses, including suicide risk assessments.
Practice in accordance with College of Nurses of Ontario practice standards, the Canadian Nurses’ Association Advanced Practice Nursing Framework, and legislative requirements.
Support a healthy workplace that embraces diversity, encourages teamwork, and complies with regulatory and legislative requirements.
Work day and evening hours as required.

Requirements

Registered with the College of Nurses of Ontario as a Registered Nurse in the Extended Class [RN(EC)] with a specialty certificate in either Adult or Primary Care.
Current BCLS.
Minimum of two years’ experience, or demonstrated expertise, working with clients with mental health and/or addiction issues.
Demonstrated skills and experience in working with diverse Black youth and communities.
Lived experience as a member of Ontario’s diverse Black communities.
Demonstrated ability to work independently and as part of an interprofessional team.
Ability to establish and maintain effective therapeutic relationships with clients with complex needs.
Excellent documentation, organizational, and interpersonal skills.
Ability to work effectively in an integrated clinical research environment with standardized data collection.
Ability to work in a dynamic and changing interprofessional environment that supports best practices, innovation, accountability, and team collaboration.
Knowledge of community resources for African, Carribean and Black youth in mental health and substance use treatment.
Demonstrated knowledge of mental health and addictions.
Strong understanding of the clinical standards related to Nurse Practitioner scope of practice.
Experience working with clients from diverse ethno-racial and cultural backgrounds.
Ability to collaborate with internal and external partners to support excellence in client care, education, and research.
Strong organizational, decision-making, and problem-solving skills.
Computer skills, including word processing, electronic communication, and workload measurement systems.
Ability to work flexible hours.
