About the Role
Sun River Health is seeking a full-time Floating Outreach Worker/Driver to engage young men of color who have sex with men, transgender youth, and at-risk youth in low-threshold services. This role involves conducting outreach on a Mobile Medical Unit and at events, providing follow-up interventions, and maintaining logs.
Responsibilities
- Conduct culturally sensitive, alternative venue outreach to engage target populations in low threshold services.
- Provide services in person on the Mobile Medical Unit and at events.
- Conduct follow-up via phone and internet.
- Maintain medical lab logs and outreach MMU logs.
- Identify new outreach locations.
- Collect biopsychosocial assessment data.
- Participate in outreach/field events.
- Provide follow-up interventions including phone calls and escorts to participants needing case management and supportive services.
- Maintain up-to-date client records in accordance with grant and HIPPA protocols.
- Promote YAP program through various outlets including internet/social media, in-reach and out-reach activities.
- Attend and participate in continuing educational programs, seminars, etc.
- Provide testing on a daily basis as clients must complete DOH HIV testing training.
- Work in-kind as correlates with the Thrive by Safe Space and field events.
