About the Role
The Care Manager II (CMII) is responsible for assessing clients with multiple medical and psychosocial needs, planning and monitoring services, and ensuring the provision of quality care to help aging adults and adults with disabilities maintain their quality of life.
Responsibilities
- Conducts comprehensive assessments and on-going re-assessments of the client including psychosocial, physical and mental health, environmental and spiritual needs.
- Writes comprehensive assessments and develops client-centered Community Living Plans based on assessment information.
- Conducts home visits, acute hospital & skilled nursing facility visits, and escorts clients to appointments.
- Identifies, arranges for, and monitors appropriate community services using knowledge of Medicare, Medi-Cal, and other entitlement programs.
- Establishes and maintains care management relationships, offering respect, dignity, and support.
- Provides crisis intervention, advocacy, problem solving, and therapeutic interventions.
- Meets with clients at least monthly, reviewing and modifying their Community Living Plan.
- Documents case management activity via progress notes within 24-48 hours.
- Maintains required paperwork and follows a charting system for continuity of care.
- Evaluates client Purchase of Service needs and follows up on service provision.
- Educates clients and their support network about resources.
- Establishes and maintains open communication with community providers, maintaining confidentiality.
- Monitors the quantity and quality of services provided by other involved providers.
- Evaluates clients' ability to remain safely at home and coordinates placement as appropriate.
- Terminates clients when appropriate in collaboration with the client, caregiver, and services.
- Participates in research studies and data collection.
- Participates in and promotes Continuous Quality Improvement efforts.
- Attends and participates in team and program meetings.
- Utilizes supervision appropriately and maintains open communication.
- Incorporates the ethical and legal standards of the National Association of Social Workers.
- Understands and applies the regulatory and procedural requirements of the Institute on Aging.
- Attends continuing education and in-service training.
- Performs other reasonably related responsibilities as assigned.
Requirements
- Russian speaking required.
- One year working with disabled adults and/or older adults required.
- Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
- Experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders highly desired.
- Exceptional communication and presentation skills relating to functionally impaired adults and older adults, their support systems and teams of health professionals.
- Demonstrates case management skills and experience in the community health care delivery system.
- Detail oriented with good problem solving skills and the ability to prioritize multiple tasks.
- Computer literacy required.

Experience Level

- Masters level degree or BA/BS with 2 years relevant experience
Education Level
- M.S.W. or another appropriate Masters level degree (e.g., MPH, MFT) with additional or specialized work experience (psychology, counseling, geriatrics)
- BA or BS in Social Work or another appropriate major with a minimum of two (2) years of relevant social work experience and demonstrated autonomous work capabilities
Salary/Compensations
- $85,759 - $102,495
