About the Role
The Case Manager will provide supportive case management services to residents of The Doe Fund's newest permanent supportive housing for homeless individuals living with HIV/AIDS. This role is crucial in ensuring residents' physical and emotional security, well-being, and independence within a community environment.
Responsibilities
- Maintain a caseload and provide supportive case management services.
- Maintain resident records in compliance with agency and funding standards.
- Conduct and review assessments.
- Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency.
- Monitor and ensure residents are linked to healthcare providers.
- Provide counseling and crisis intervention as necessary.
- Assist residents in securing and maintaining benefits.
- Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs.
- Facilitate groups and activities that promote health and independent living skills development.
- Monitor and assist residents with rent payments and budgeting.
- Conduct regular unit inspections.
- Educate residents about approaches to mental health.
- Attend ongoing training to enhance the services provided to the residents.
- Assist with the completion of reports as needed.
- Perform other duties as assigned.
Requirements
- Must have a working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness.
- Excellent case management skills, including knowledge of benefits in the NYC Metro area.
- Sensitivity to community issues and needs.
- Outstanding interpersonal and communication skills, both oral and written.
- Strong ethical standards, confidentiality, and personal integrity.
- Flexibility with schedule to cover evening activities.
- Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint).
