About the Role
Deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth, focusing on comprehensive geriatric assessment, cognitive screening, medication review, advance care planning, and chronic condition management. You will collaborate with the CMO and receive full training and clinical support.
Responsibilities
- Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk
- Provide medical follow-up visits for chronic disease management and ongoing care
- Perform cognitive screening and staging; escalate for further assessment where indicated
- Prescribe and manage GLP-1 therapy for appropriate patients
- Conduct advance care planning conversations
- Document in our EHR to support accurate Medicare billing
Requirements
- Active, unrestricted NP license in New York
- Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging
- Active DEA registration
- Experience with Medicare documentation and E/M coding
- Comfort with telehealth and EHR documentation
- Reliable high-speed internet and a private, HIPAA-compliant workspace
