About the Role

The Forensic Assertive Treatment (FACT) team provides evidence-based, mobile, community-based mental health treatment to individuals age 18 and up with serious mental illnesses, criminal legal involvement, co-occurring substance use disorders, and high utilization of crisis services. The FACT Team Leader directs and supervises staff, leads meetings, and coordinates administrative functions, collaborating with the FACT psychiatrist for clinical direction to ensure effective treatment and rehabilitation services.

Responsibilities

  • Oversees clinical and administrative operations of the FACT Team, including daily team meetings, clinical meetings, documentation timeliness and quality, intakes, discharges, after-hour emergency line, service dollar usage, and staff schedules.
  • Ensures the team delivers culturally and developmentally appropriate, trauma-informed, recovery-oriented services and effectively applies motivational interviewing.
  • Provides regular clinical supervision, training, and professional development opportunities for team members.
  • Provides direct clinical care and services to consumers in the community when needed.
  • Ensures the team meets performance outcomes and operational standards per New York State Office of Mental Health (OMH) licensing and New York City Department of Health and Mental Hygiene (DOHMH) contracting standards.
  • Oversees team’s incident reporting procedures according to OMH standards and ensures adherence to Incident Review Committee (IRC) policies and procedures.
  • Conducts utilization review and quality assurance reviews in collaboration with the Quality Assurance department to ensure compliance with New York State OMH guidelines, New York City DOHMH operating standards, and HIPAA confidentiality regulations.
  • Consults with senior clinical leadership on complex, high-risk cases, and team and personnel issues.
  • Coordinates monthly Medicaid billing using the electronic medical record to select billable progress notes and ensure billing targets are achieved.
  • Performs other duties as required by ACT Leadership and executive staff.

Requirements

  • LCSW, LMSW, LMHC, RN, or Licensed Psychologist (PsyD/PhD)
  • Two years of supervisory experience
  • At least three years of experience providing services to adults with serious mental illness
  • Experience working in community mental health settings providing in-vivo community services
  • At least two years of experience working on a multidisciplinary team
  • Experience working with and knowledge of the needs and challenges faced by individuals involved in the criminal legal system

Skills

  • Fluency in Spanish strongly preferred
