About the Role
Project FIND seeks a Social Worker to provide case management, counseling, and support services to older adults at the Hamilton and Hargrave House locations. This role involves conducting assessments, developing service plans, and assisting clients with housing, benefits, and daily living challenges.
Responsibilities
- Conduct intake and assessment screening, assessing mental and physical health needs and supports, and financial eligibility for benefits and entitlements.
- Conduct home, office, and community visits to address unmet needs.
- Implement constructive short and long-term interventions through counseling, education about counseling and physical health supports, long-term planning, and assistance with benefits and entitlements.
- Provide orientation, support, and services necessary to assist tenants to remain permanently housed in a safe environment.
- Assist with income affidavits, housing applications, and recertifications in collaboration with Property Management.
- Assist in collecting rent as needed.
- Develop and create training events to educate clients on topics such as self-care, health and wellness, and legal planning.
- Lead support groups, providing emotional support and psychoeducation on aging in place, health and wellness, and self-care.
- Develop and create internal and external community building events and opportunities to facilitate activity and socialization.
- Provide conflict resolution and engage in tenant concerns in matters requiring enhanced intervention.
- Provide timely documentation and maintenance of client files and progress notes.
- May provide supervision, training, and support to paraprofessional staff, including students.
- Perform other related tasks as requested or assigned.
Requirements
- Bachelor's degree in human services or related field required.
- MSW/LMSW preferred with at least 2 years relevant experience in social services.
- Knowledge of benefits and entitlements is a plus.
- Experience working with older adult populations and families preferred.
- Bilingual Spanish fluency preferred.
- Strong computer skills, including Microsoft Office.
- Ability to provide accurate and timely documentation.
- Time management skills to coordinate multiple responsibilities and services.
- Ability to work independently and as part of a team.
- Ability to work in both a home and office environment.
- Maintain client and agency confidentiality.
- Excellent interpersonal, public speaking, and written communication skills.
- Ability to communicate effectively and appropriately with other team members, community agencies, and clients with diverse opinions, values, characteristics, and cultures.
