About the Role
Improve patient health through an innovative, patient-focused culture dedicated to making a difference in the communities served.
Responsibilities
- Provide diversified support for performance improvement, regulatory, accreditation, patient safety, and health plan quality activities.
- Conduct performance improvement related case reviews, focus studies, and other initiatives.
- Lead and/or facilitate multidisciplinary teams in identifying, prioritizing, and implementing performance improvement projects.
- Report unbiased data, findings, and summaries to the QAPI Committee quarterly or as needed.
- Participate in healthcare failure modes, effects analysis, and root cause analysis activities.
- Act as an internal consultant and collaborate with medical staff and departments regarding regulatory, accreditation, and continual readiness efforts.
- Collect and process quality data to identify areas for improvement.
- Monitor clinical performance to ensure patient safety and promote quality outcomes.
- Prepare and analyze data for performance improvement, accreditation, and safety-related committees.
- Assist with all components of regulatory and other quality programs, accreditation preparedness, and continual readiness.
- Maintain current knowledge of regulatory quality program specifications, accreditation process, and survey process.
- Assist with Virtual Survey and/or CMS Validation activity preparation, completion, and follow-up.
- Complete organizational mock surveys and tracer activities.
- Assist departments with quality program activities and accreditation-related activities.
- Lead or facilitate performance improvement teams/processes related to quality programs and accreditation/survey issues.
- Conduct audits and focus studies to evaluate compliance.
- Lead and serve as a liaison in clinical quality measures and core measure data collection and analysis.
- Function as a key facilitator of performance improvement throughout the organization.
- Assist departments with PI education, initiatives, and opportunities.
- Complete organizational focus studies, case reviews, and audits, including analysis of results and identification of improvement strategies.
- Participate in organizational performance improvement initiatives at the assigned level.
- Maintain open communication within the department regarding quality and safety issues.
- Participate in activities designed to improve patient safety.
- Serve as a department liaison for initiatives such as restraint management and National Patient Safety Goals.
- Participate in activities designed to improve quality of care, reduce medical errors, patient falls, and medication errors.
- Report barriers to success in a timely manner.
- Act as a member of quality-related committees.
- Demonstrate ongoing competence and upgrade knowledge through continued education.
- Attend educational programs to attain current information.
- Review and forward pertinent reading material addressing PI, quality, accreditation, or patient safety issues.
- Support organizational strategies and initiatives related to quality improvement, accreditation, and patient safety.
- Perform similar or related duties as requested or directed.
- Provide education on quality, accreditation/survey process, and continual readiness topics (for Nursing Home based Quality Specialist).
- Complete other quality program and survey-related assignments as directed (for Nursing Home based Quality Specialist).
- Lead or participate in patient safety activities and/or survey review (for Nursing Home based Quality Specialist).
Requirements
- Minimum 1 year of experience in healthcare or a healthcare related field.
- Experience with Joint Commission standards and survey process, CMS, NYS DOH regulations, and audit process, preferred.
