About the Role
The London HART Hub is a low barrier recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs. It connects clients with a continuum of care, including transitional supportive housing, basic needs, healthcare, peer supports, employment, mental health and addiction services, and more. Relief Addiction and Mental Health Workers assist clients with integrated care planning, system connection and navigation, onsite support with basic needs, and mental health and addiction supports.
Responsibilities
- Provide intake, screening, assessment and treatment planning
- Support clients on their journey towards physical/mental health improvement
- Ensure clients basic needs are being met (e.g., provision of food, showers, hygiene and a place to rest)
- Provide referrals to other supports and resources to maintain overall wellness
- Assist with the development of care plans, navigating local services, and engaging in proactive crisis intervention
- Provide one-on-one and group counselling sessions with a strengths-based wellness approach
- Ensure client record is updated with changes in circumstances, housing, medications, health conditions, diagnoses, etc
- Provide navigation assistance, connection to resources, and collaboration with established justice supports for those involved with the criminal justice system (pre-release support, probation or bail)
- Provide accessible options for communication for clients with varying needs and literacy levels (AODA, multiple languages, sign language, audio translation and referral to language classes)
- Assist with transportation needs upon intake, including obtaining travel assistance such as bus passes or other forms of travel, leveraging existing funding support
- Provide integrated care plans that encompass acute and primary care, mental health, addictions, income, transportation and safety
- Provide support for additional HUB sites as required
- Provide guidance with income planning, applications (e.g., ODSP, OW, CPP), and tax filing
- Provide connections to employment support programs
- Provide strategies to access barriers, build confidence and improve employability
- Provide 24/7, on-site support to help individuals maintain stable housing at HART Hub
- Facilitate ongoing conversations around housing goals to secure suitable housing
- Provide rapid connection to housing stability resources
Requirements
- Completed post-secondary education in a related field of study
- Valid G driver's licence and access to a vehicle with at least $2 million liability insurance to occasionally transport clients
- Ability to attend work regularly
- Police Vulnerable Sector Check completed in the last six months
Skills
- Addictions or problematic substance use
- Mild, moderate and serious mental illness
- No fixed address
- Residential/community housing experience
- Familiarity with the Residential Tenancies Act
- Crisis intervention/management
- Crisis resolution
- Working with individuals with an ABI or Developmental Disability
- Working in a community-based setting
- First Aid & CPR
- ASIST
- CPI certifications
- Bilingual in both official languages

Salary/Compensations
- Starting rate $27.14 per hour (Level 1 – Direct Service)
- Wage differential $30.01 per hour for Level 1 – Direct Service, Regulated
- Up to $34.41 per hour for non-regulated (Level 8)
- Up to $37.26 per hour for regulated (Level 8)
- Premiums of $2.00 for eligible Evening/Night and Weekend Shifts
