About the Role
The community habilitation specialist will work directly with individuals in program activities, both in their homes and in the community. Assist individuals in skills development by implementing individual specific and general programs. The Community Habilitation Specialist will interact professionally and ethically, understand and implement service plans, and complete all required documentation clearly, legibly, and in a timely manner.
Responsibilities
- Participate in the life of the individual in their home to foster a person-centered atmosphere conducive to growth and independence.
- Commit to person-centered supports and honor individual personality, preferences, culture, and strengths.
- Promote and protect the health, safety, and emotional well-being of individuals.
- Assist in planning and implementation of the program.
- Maintain daily Community-Habilitation bi-weekly progress reports, datasheets, timesheets, and other required documentation.
- Document each training session in appropriate databases and report progress and barriers.
- Attend and participate in mandatory staff and in-service training and meetings.
- Know and protect individual rights.
- Maintain contact with Program Supervisors and individuals/families regarding schedule changes, lateness, absences, and other issues.
- Communicate relevant information to Program Supervisors verbally and in writing.
- Complete documentation electronically as required.
- Follow up with electronic visit verification (EVV) on assigned dates.
- Follow all specified policies and procedures for managing behaviors, emergencies, and reporting incidents.
- Accompany and advocate for the individual in the community.
- Travel to assigned borough.
- Act as a role model for consumers through appropriate behavior and assistance.
- Meet with the assigned individual a few times per week.
- Provide personal care consistent with participant's skill level and habilitation plan.
- Support the mission and vision of the organization.
- Perform other related duties as requested by supervisor or the organization.
Requirements
- Must have a personal computer device (smartphone or tablet) capable of performing required documentation using company software.
- Unrestricted NYS Driver's License (Preferred).
- Proof of car insurance needed if client's goals require vehicle use.
- CPR Certification (Preferred).
