About the Role
The Case Manager/Mental Health Counselor will deliver supportive case management services to HASA-funded residents within the Villa House Permanent Supportive Housing Program, which provides long-term residential care and 24-hour support services for homeless men and women living with HIV/AIDS. The program aims to maximize residents’ independence while ensuring their physical and emotional security within a supportive community environment.
Responsibilities
- Maintain a caseload and provide regular case management services.
- Maintain resident records in compliance with agency and funding standards.
- Conduct and review assessments.
- Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency.
- Monitor and ensure residents are linked to healthcare providers.
- Provide counseling and crisis intervention as necessary.
- Assist residents in securing and maintaining benefits.
- Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs.
- Facilitate groups and activities that promote health and independent living skills development.
- Monitor and assist residents with rent payments and budgeting.
- Conduct regular unit inspections.
- Educate residents about approaches to mental health.
- Compile resources on mental health information and treatment strategies.
- Attend ongoing training to enhance the services provided to the residents.
- Assist with completion of HASA’s weekly and monthly reports as well as any other relevant reports.
- Perform other duties as assigned.
Requirements
- Master’s Degree in Social Work or related field.
- At least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness.
- At least 5 years of experience in the mental health field.
- Knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issues.
- Solid computer skills – internet, Microsoft Office, and charting database.
- Great organizational, writing, and communication skills.
