About the Role
The Case Management Supervisor is a key member of the human service leadership team, responsible for overseeing the development and execution of service and care plans for residents. This role aims to create a supportive environment that fosters stabilization, independence, self-reliance, and skills development, while leading a team of Case Managers and coordinating community resources.
Responsibilities
- Ensure residents receive quality and appropriate services per organizational principles and program requirements.
- Review service and care plans to ensure they are customized to resident needs, aligned to biopsychosocial assessments, and promote independence and long-term stability.
- Develop partnerships and service agreements with external providers for issues such as immigration, legal, health, mental health, childcare, public benefits, employment, and education.
- Ensure residents are actively engaged in public benefits and city programs.
- Ensure school-age children in Family Shelters receive transportation, school transfers, and ESL/ELL/IEP services as needed.
- Develop and maintain a resource network for referrals.
- Ensure Case Managers leverage external services and advocate with providers to secure services.
- Ensure residents maintain healthy hygiene and housekeeping.
- Engage residents with behavioral issues compromising health and safety with Social Workers and external resources.
- Review discharge plans to ensure continued enrollment in public benefits and linkages to community-based organizations.
- Review performance and productivity of Case Managers, providing constructive feedback, guidance, and technical assistance.
- Provide performance evaluations.
- Troubleshoot difficult resident cases and intervene in special needs cases.
- Conduct regular 1-on-1 meetings with team members and update performance goals in Paycom.
- Ensure Case Managers adhere to CHI code of conduct and comply with all relevant rules and regulations.
- Document interactions and communications with residents in CARES and AWARDS.
- Perform other duties as assigned by supervisor and Program Director.
Requirements
- Bachelor’s degree in social work or psychology required.
- Five (5) years of experience plus two (2) years of supervisory experience in social services for clients.
- Minimum of seven (7) years of case management experience and three (3) years of supervisory experience in the past ten (10) years may be substituted for educational requirement.
- Professional verbal and written communication skills.
- Proficiency in English required.
- Professional competency in MS Word, Excel, Outlook and the internet.
- Ability to formulate simple financial budgets and coach budget management skills.
- Ability to prioritize, make appropriate decisions and judgment calls.
- Experience working with homeless individuals and people living with health and/or mental health issues and substance use.
- Ability to engage residents.
- Extensive experience with service and care plan development and implementation.
- Experience in building and sustaining partnerships and collaborations with external service providers and community-based organizations.
Skills
- Multi-lingual and Spanish proficiency prioritized.
- Experience with CARES, HOME, CAPS, and AWARDS prioritized.
