About the Role

Direct and provide social work services, including case assistance, coordination, and management at the Helen Harris Residence, a Section 2020 with 108 units for elderly and disabled seniors. Coordinate with Staff of Two Bridges regarding care coordination. Determine client needs and preferences, develop individualized care plans, identify resources, and monitor care plans, adapting them as client needs change.

Responsibilities

  • Provide case management services in Chinese and English, including information and referral, advocacy, benefit and entitlement assistance, care plan development and maintenance, financial planning, and assessment/referral for mental health issues, family issues, and cross-cultural barriers.
  • Provide outreach and home visits.
  • Coordinate services with health professionals.
  • Coordinate care and services clients receive from various auspices, including home health care, benefit & entitlement agencies, and legal/financial service providers.
  • Provide annual housing recertifications.
  • Maintain compliance with HUD guidelines.
  • Participate in trainings.
  • Compile and submit reports and vouchers regularly.
  • Work with management company and building staff on necessary issues.
  • Participate in regular administrative staff and senior service coordination meetings.
  • May assist with senior service-wide events.
  • Participate in regular supervision with the Assistant Executive Director.
  • Perform other appropriate duties as assigned.

Requirements

  • Bachelor's degree in a Human Services field required; Master's degree preferred in social work, counseling, psychology, gerontology, or a closely related discipline.
  • Bilingual proficiency in English and Chinese required.
  • Three years of experience working with older adults preferred.
  • Sensitivity to and understanding of the needs of older adults.
  • Ability to handle complex and difficult situations professionally, maintaining a calm and professional demeanor.
  • Ability to develop collaborative relationships with designated community groups.
  • Excellent computer skills, including experience with data management systems, and willingness to learn other programs.
  • Strong verbal and written communication skills.
  • Highly organized and detail-oriented, able to plan, manage, and complete multiple concurrent projects with short deadlines.
  • Ability to work independently.
  • Strong commitment to the mission of HMH.
