About the Role
Wraparound Maryland is a non-profit mental health services company dedicated to inspiring, empowering, and actuating individuals' unique visions and goals through a holistic, person-centered approach. This role is integral to the company's sustainability and goal achievement.
Responsibilities
- Provide intensive case management services using a family-centered approach.
- Assist clients in achieving goals related to stability, permanent housing, employment, and self-sufficiency.
- Provide structure, direction, and service linkage to connect clients with community resources and overcome barriers to housing.
- Complete in-depth needs assessments and develop detailed Individual Service Plans (ISPs) with specific goals and action plans.
- Focus ISP goals on increasing income, life skill development, employment/education, physical and mental health, fiscal management, childcare, and parenting.
- Meet bi-weekly with clients to track progress, provide direction, and offer counseling.
- Facilitate service linkages for clients in areas such as health, mental health, education, employment, benefits, and parenting assistance.
- Monitor client attendance at on-site classes for life skills, parenting, and employment readiness.
- Maintain accurate and up-to-date case records and document services delivered.
Requirements
- Prolonged periods of sitting at a desk and working on a computer.
- Ability to lift 15 pounds at times.
- Ability to access and navigate each department at the organization’s facilities.
