About the Role
Provide nursing care to patients in a home setting, assessing and educating patients and caregivers to enhance quality of life while adhering to agency standards and physician orders.
Responsibilities
- Demonstrate efficient teamwork with staff.

- Perform nursing procedures according to agency policy and procedures.
- Work under the direction of a RN.
- Monitor patient progress using observation, assessment, and evaluation skills.

- Report adverse findings to physician and RN.
- Follow Medicare and Medicaid regulations.
- Coordinate and monitor patient care and services.
- Comply with HIPPA regulations.
- Follow infection control policy.
- Document skilled visits according to Medicare guidelines.
- Maintain patient records according to policy and procedures.
- Participate in in-services, workshops, seminars, and self-study courses annually.
Requirements
- Licensed Vocational Nurse.
- Minimum of 2 years experience in a healthcare setting.
- Excellent oral and communication skills.
