About the Role
The Care Coordinator serves a designated caseload of clients in our Health Homes Program, focusing on meeting all client needs through day-to-day activities and playing an integral part in the care management team.
Responsibilities
- Provide intensive care coordination activities for clients.
- Assure members have access to, engage in, and retain needed services as defined in the member's care plan.
- Advocate for client services by phone and in-person as needed.
- Perform home visits and follow-up to monitor services and client status.
- Participate in case conferences and training.
- Maintain relationships with service providers.
- Maintain accurate and timely documentation including all documents included in client charts, records, and billing activity logs.
- Attend supervision with the Care Manager as per agency policy.
Requirements
- Requires a Bachelors Degree or a High School Diploma with 2 years of care management experience.
- Experience working with the homeless and HIV populations required.
- Bilingual preferred, with good verbal skills in Spanish.
- Exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.
- Clean valid driver's license required.
