About the Role
Work with the CN Guidance Youth ACT Team to deliver services to youth diagnosed with severe emotional disturbance (SED) and their identified families, whose needs have not been met by more traditional service delivery approaches.
Responsibilities
- Provide Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between Youth ACT and other systems.
- Provide culturally sensitive and developmentally appropriate treatment to Youth and their families, including Assertive Engagement, Case Management, Assessment, Child and Family Services Planning, Individual, group, and/or Family Counseling/therapy, Family Psychoeducation, Individual Psychoeducation, Psychosocial Rehabilitation, Crisis Intervention Services, and Vocational/Educational Services.
- Maintain case records and complete documentation on time and in compliance with federal, state, and agency regulations.
- Provide intensive, in-person, community-based services calibrated to each individual's immediate needs and adjusted in real time based on ongoing assessment, in full compliance with OMH regulations.
- Be available on an as needed basis 24 hours a day, 7 days a week, including periodic afterhours coverage on the phone and in person as needed.
- Assist recipients in developing appropriate expectations from treatment.
- Strengthen the ability of the recipient and their identified family to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination process.
- Exchange information with other agencies that benefits the recipient or ensures compliance with regulations and coordination of services.
- Utilize and understand the value of supervision to increase practice skills, promote better services for the recipient, and preserve accountability to the Agency.
- Participate in staff development, providing staff supervision and training.
- Actively engage in agency-wide trainings and work groups, as well as NYS specific training on and for the Youth ACT model.
- Facilitate sharing of data with individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress.
- Complete QI Training during onboarding and participate in QI refresher training as needed.
- Cultivate a culture of inclusion for all employees that respects their individual strengths, views, and experiences.
- Adhere to all safety protocols and procedures to ensure a safe working environment for all employees.
- Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.
- Possess knowledge or experience regarding health and safety protocols to ensure a safe work environment for all employees.
- Willing to transport clients in an agency vehicle on an as needed basis.
- Assist and coordinate in planning for community integration trips with Youth ACT Recipients.
- Perform other duties as assigned.
Requirements
- Two years' experience working with the chronically mentally ill (Masters level).
- Demonstrated competency in substance abuse, family support, or vocational/educational rehabilitation through at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.
- Willingness to transport clients in an agency vehicle on an as needed basis.
