About the Role
Community Living Staff 2 works as an effective member of the team, assisting each resident with carrying out their service plan goals and objectives using a person-centered approach.
Responsibilities
- Provide teaching, monitoring, and recovery-oriented supports in accordance with OMH 595, including rehabilitative services such as Assertiveness/Self-Advocacy Training, Community Integration/Resource Development, Daily Living Skills, Health Services, Medication Management/Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.
- Assist the Medication Coordinator with supervising resident medications per physician's orders, agency policy, and NYS Office of Mental Health regulations.
- Maintain accurate and timely documentation in resident medication records.
- Communicate resident mental and physical status information to the Residential Services Coordinator, on-site Supervisor, or provider as needed.
- Complete basic documentation in resident files regarding psychiatric or medical concerns, progress, or other significant events.
- Ensure daily tasks and assignments are completed as directed, including preparing resident rooms, orienting new residents, and assisting with discharges and transfers.
- If working the overnight shift, complete rounds at least 4 times per shift to monitor the program and intervene in resident disputes, including census checks.
- Work with Overnight security to ensure resident safety and well-being, including building/grounds observation, enforcing visitor hours, securing doors, restricting unwanted visitors, and monitoring security cameras.
- Share information with supervisors or administrative staff regarding problems or concerns occurring during the shift.
- Assist residents in maintaining the upkeep of their personal rooms.
- Coordinate and participate in resident recreational activities.
- Respect and maintain resident confidentiality and demonstrate a caring, positive attitude toward residents, staff, family members, guests, and service providers.
- Attend staff meetings and trainings as required.
- Act as a member of the residential team for each resident, providing input for treatment team meetings and Continued Stay Reviews.
- Provide safe transporting of residents when needed.
- Perform other duties necessary for the effective operation of the program.
Requirements
- Minimum of High School diploma or GED.
- Ability to demonstrate verbal and written skills necessary to carry out services for program success.
- At least one year of experience in a residential setting working with people in various stages of recovery from mental illness preferred.
- Valid New York State driver’s license that meets agency’s clean driver record policy.
- Must be at least eighteen (18) years of age.
