About the Role

Care Coordinators link adults and children with chronic behavioral health and medical conditions to essential services, inspiring members to optimize their health outcomes. This role involves assessing needs, developing care plans, and coordinating health and community services, primarily in the field.

Responsibilities

  • Integrate medical, specialized, and behavioral health services, along with social support and educational services.
  • Periodically assess a member’s medical and behavioral health needs and compliance with recommended treatments.
  • Collaboratively develop an Individualized Care Plan (ICP) with the member, their family/caregivers, and service providers.
  • Provide required care management services.
  • Track all specialty medical, behavioral, and support service referrals using provided Health Information Technology (HIT).
  • Ensure members access, engage in, and retain needed services as defined in their ICP, including acute and primary medical care, preventative services, home health care, chemical dependency services, behavioral health services, community social support, housing, state and federal entitlements, educational services, and involvement with child welfare, juvenile justice, or criminal justice institutions.
  • Provide outreach services to members to increase access to services.
  • Respond to member information and referral questions.
  • Reassess the need for ongoing care coordination services.
  • Complete all required documentation.
  • Share knowledge and experience with team members to support overall service provision.
  • Carry an agency-provided cell phone.
  • Respond to member crises during and occasionally outside of regular business hours.
  • Perform other duties as assigned.

Requirements

  • Experience working in interdisciplinary teams.
  • Experience providing care management or care coordination in a medical or behavioral health environment.
  • Experience working with the chronically ill.
  • Ability to travel within New York City carrying equipment weighing up to approximately 10 pounds.
  • Ability to sit (60% of the time), stand (20% of the time), and travel to and from appointments using varied public and private transportation options (20% of the time).

Experience Level

  • Two years of experience in providing direct services, or a substantial number of case management services, to mentally disabled or chronically ill or homeless individuals, or children which complex social or healthcare needs (if applicable degree/certification is met)
  • Five years’ experience working with an applicable population (if Bachelor’s Degree, Associate’s Degree or High School Diploma/GED in another discipline)

Education Level

  • Bachelor's degree with a major or concentration (minimum of 24 credits) in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing.
  • NYS teacher's certification for which a bachelor's degree is required.
  • NYS licensure and registration as a Registered Nurse and a bachelor's degree.
  • Bachelor’s Degree, Associates Degree or High School Diploma/GED in another discipline.
