About the Role
Horizon Health Services is seeking passionate individuals to improve perinatal mental health and support women and birthing individuals facing mental health and substance use challenges. Responsibilities
- Maintain a primary caseload consisting of pregnant and postpartum patients, women and families struggling with loss or fertility challenges, as well as providing therapy to additional populations as needed.
- Facilitate groups and individual therapy for our Perinatal Intensive Outpatient Program.
- Attend trainings via our new state-of-the-art learning management system to gain necessary knowledge.
- Conduct comprehensive patient assessments and develop treatment plans that identify patient needs and strengths.
- Build effective therapeutic relationships by developing rapport and hope with patients of diverse backgrounds.
- Participate in weekly one-on-ones with your supervisor and team meetings to discuss case presentations.
- Utilize evidenced based practices such as CBT, DBT, and MI to serve as a catalyst for change and improved quality of patients’ lives.
- Creatively problem solve with patients around practical obstacles to increase whole body wellness.
- Collaborate with psychiatrists, care coordinators, and family members to increase patient support network.
- Facilitate group therapy as part of our perinatal intensive outpatient program.
Requirements
- Completed Master’s degree in a program that will grant licensure in the state of New York.
- Certification as a PMH-C or at least 2 years of experience working in the maternal mental health/perinatal field.
