About the Role
Provides general nursing care to patients, families, and caregivers in Medical Group practices. Assists and advocates for patient and family health within a patient-centered care model. Collaborates with physicians, registered nurses, and multidisciplinary teams. Performs case finding, health coaching, and supportive care under the direction of a registered nurse or licensed provider, adhering to NYS Nurse Practice Act and Mohawk Valley Health System standards.
Responsibilities
- Gather patient measurements and collect patient data.
- Record and report patient findings.
- Prepare patients for examination and treatment.
- Assist specialty providers with examinations, treatments, and procedures.
- Perform supportive care tasks under the clinical direction of a licensed health care provider or registered professional nurse.
- Support self-care, self-management, and the established plan of care.
- Utilize standard precautions at all times.
- Maintain equipment, supplies, inventory, and a safe work environment.
- Maintain technical knowledge through educational workshops, in-services, publications, and committee meetings.
- Engage in quality improvement initiatives and continuing education.
- Maintain cooperative relationships within the healthcare team through effective communication.
- Respond to patient requests and communicate effectively to the appropriate licensed health care provider.
- Perform other duties as required.
Requirements
- Graduate of a practical nursing education program.
- Current New York State licensure as a Licensed Practical Nurse.
- Basic Life Support Certification - must receive BLS certification within 6 weeks of hire date.
