About the Role
Halcyon Home is seeking a dedicated and compassionate Home Health Physical Therapist to support our growing geriatric home health population in South Austin and surrounding areas. We offer competitive pay and wonderful team support.
Responsibilities
- Performs ongoing clinical assessment, including OASIS assessments, to identify patient needs, determine level of functioning, and appropriateness for home care.
- Develops and evaluates the plan of care in partnership with the patient, representative, and caregiver(s).
- Meets mandatory continuing education requirements.
- Assists and instructs patients and families in safe transfer techniques and the use of equipment such as walkers, wheelchairs, and canes.
- Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
- Provides patient, caregiver, and family counseling and education.
- Applies infection control and standard precautions to prevent disease transmission.
- Documents clinical notes, phone orders, and other clinical record documentation in a timely manner.
- Revises the plan of care based on evaluation and ongoing assessment data.
- Communicates with physicians and other healthcare practitioners regarding the plan of care.
- Accepts clinical assignments consistent with education and competence.
- Provides services ordered by the physician and oversees other team members when applicable.
- Provides supervision and guidance to physical therapy assistants and home health aides.
- Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders.
Requirements
- Current PT license
- Knowledge or experience utilizing Homecare Homebase EMR is a plus
- Experience in home health or related healthcare setting preferred
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively.
