About the Role

The Entitlements Coordinator administers public benefit programs for individuals supported by ADAPT Community Network, ensuring timely application, maintenance, renewal, and continuity of benefits while adhering to regulations and agency policies. This role involves collaboration with various internal teams and external government agencies to support daily entitlement operations.

Responsibilities

  • Coordinate the application, verification, renewal, recertification, and ongoing maintenance of public benefits, including SSI, SSDI, Medicaid, MSP, SSP, SNAP, and other applicable Federal, State, and local benefit programs.
  • Verify benefit eligibility for new admissions and support continuity of benefits throughout service delivery.
  • Prepare, review, and submit required documentation to Federal, State, and local agencies in accordance with established deadlines.
  • Monitor pending applications and follow up with government agencies to facilitate timely benefit determinations.
  • Track renewals, recertifications, and required documentation to help prevent interruptions in benefits and services.
  • Coordinate routine Medicaid renewals, eligibility updates, and supporting documentation.
  • Assist with Medicaid conversions, NHED documentation, and other eligibility processes under the guidance of the Senior Entitlements Coordinator.
  • Obtain and maintain client bank account information for new residential admissions in accordance with agency policy.
  • Monitor financial information that may affect ongoing public benefit eligibility and notify the Senior Entitlements Coordinator of significant changes.
  • Communicate with the Social Security Administration, OPWDD, HRA, LDSS, Managed Care Organizations, and other government agencies regarding entitlement matters.
  • Respond to requests for information and obtain documentation necessary to support benefit eligibility.
  • Monitor and respond to notices, correspondence, and requests received from government agencies.
  • Escalate complex eligibility determinations, appeals, overpayments, trust-related issues, and regulatory concerns to the Senior Entitlements Coordinator.
  • Utilize agency and government systems, including ePACES, OPWDD CHOICES, Medicaid Client System (MCS), ACCESS HRA, Social Security Business Services Online (BSO), and other systems used in entitlement administration.
  • Maintain complete, accurate, and confidential entitlement records in accordance with agency policies and regulatory requirements.
  • Document case activity, correspondence, applications, renewals, and contacts with government agencies.
  • Prepare reports, summaries, and supporting documentation as requested.
  • Ensure timely filing and retention of required entitlement documentation.
  • Collaborate with Residential Directors, Program Directors, Finance, Admissions, Care Management, and interdisciplinary teams regarding benefit-related matters.
  • Provide courteous, professional, and responsive customer service to people supported, families, guardians, advocates, and internal stakeholders.
  • Participate in interdisciplinary meetings, departmental meetings, audits, and committees as requested.
  • Assist with departmental projects, quality assurance activities, and process improvement initiatives.
  • Maintain knowledge of applicable Federal, State, and local regulations governing public benefit programs.
  • Participate in departmental training and continuing education activities.
  • Adhere to agency policies regarding confidentiality, HIPAA, financial information, and documentation standards.
  • Perform additional duties and special projects as assigned by the Senior Entitlements Coordinator and/or Entitlements Director.
  • Maintain professionalism, accountability, and confidentiality in all aspects of work.

Requirements

  • Bachelor's degree in Social Work, Human Services, Public Administration, Healthcare Administration, or a related field required.
  • Minimum one year of experience administering or coordinating public benefits or entitlement programs.
  • Working knowledge of Social Security, Medicaid, SNAP, and related entitlement and public benefit programs.
  • Experience working with OPWDD-funded services and people supported with developmental disabilities strongly preferred.
  • Strong organizational skills with the ability to manage multiple priorities and deadlines.
  • Excellent verbal and written communication skills.
  • Ability to maintain confidentiality and exercise sound judgment.
  • Strong interpersonal and customer service skills.
  • Proficiency with Microsoft Office and database systems.
  • Ability to work independently and collaboratively across departments.
  • Ability to travel between agency locations and government offices as needed.
