About the Role
The Licensed Vocational Nurse (LVN) or Licensed Psych Tech (LPT) provides field-based nursing care, complex medical care management, and at-home care to HIV positive clients. This role is central to an integrated, mobile team, supporting health and psychiatric assessments, and developing client care plans.
Responsibilities
- Provides mobile nursing care and case management to HIV positive clients.
- Supports field-based health and psychiatric assessments and monitoring.
- Offers enhanced medication support, including dispensing and injections.
- Coordinates directly with pharmacies.
- Provides complex care management, including hospital transition support, medical referrals, and health education.
- Provides service referral, linkage, coordination, and advocacy services.
- Develops and renews clients’ individualized nursing treatment plans and multidisciplinary care plans.
- Provides clinical oversight for case management care plans.
- Coordinates with clients’ primary care provider weekly.
- Completes necessary documentation, UOS reports, and treatment plans in a timely manner.
- Documents appropriately in the electronic medical record (Epic).
- Uses multiple electronic data sources to gather patient information and track ED or inpatient utilization.
- Participates in weekly case conferences, monthly staff meetings, and in-service trainings.
- Performs other duties and tasks as assigned.
Requirements
- Candidate must be a Licensed Vocational Nurse or Licensed Psych Tech in the state of California.
- Knowledge of HIV/AIDS, STIs, substance use, and mental health issues.
- Experience working with LGBTQ populations.
- Familiarity with HIV/AIDS issues in communities of color.
- Understanding of team-based care.
- Knowledge of harm reduction philosophy, client-centered counseling, and motivational interviewing techniques.
- Must be adaptive, flexible, and a team player.
- Strong verbal and written proficiency in English.
