About the Role

Oversee a team of Case Managers to ensure families with children in temporary shelter receive necessary support and resources for self-sufficiency and independent living, in compliance with CRF, DHS, and OTDA standards.

Responsibilities

  • Ensure client documentation in CARES is completed immediately upon client arrival.
  • Provide field orientation and on-the-job training to Case Managers.
  • Create and execute plans for meeting contracted Independent Living Program (ILP) completion and placement goals.
  • Ensure Case Managers complete all mandated CRF, DHS, and OTDA tasks within prescribed timeframes.
  • Ensure Case Managers provide consistent services in accordance with CRF, DHS, and OTDA policies and procedures.
  • Monitor Case Managers' work activities through CARES reviews, physical chart reviews, and spreadsheets.
  • Conduct individual and group supervisions with Case Managers, providing documented feedback on performance.
  • Conduct performance evaluations of Case Managers per CRF, DHS, and OTDA policies.
  • Collaborate with the Social Services team to develop service plans addressing client needs in housing, substance abuse, mental health, and employment.
  • Meet with Senior Management for individual and group supervision.
  • Assist families in identifying appropriate housing resources and the application process.
  • Monitor demographic reports to guide Case Managers and ensure program compliance.
  • Attend and moderate Case Conferences with Case Managers and clients facing challenges.
  • Establish linkages with community service providers for housing, health, and social services.
  • Assist in facilitating intake, discharge, transfers, and placement into permanency.
  • Complete ILPs and other Case Management related tasks as needed.
  • Generate and distribute reports, including exit and quarterly projections, and case summaries.
  • Support the acquisition of housing vouchers and placements.
  • Deliver crisis management and intervention services to clients.
  • Support efforts to place families into permanent housing.

Requirements

  • Bachelor’s degree in Social Work required.
  • At least two years of experience in a related field; experience with multi-problem families is desirable.
  • Bilingual in Spanish.
  • Demonstrated knowledge of and proficiency in CARES.
  • Demonstrated experience working in a shelter setting and/or with high-risk families, including those with documented histories of mental health, substance abuse, Domestic Violence, child abuse/neglect.
  • Ability to work effectively with a wide range of community-based providers.
  • Working knowledge of Case Management theories and approaches.
  • Knowledge of community health services, social service support agencies, and networks.
  • Familiarity with DHS/OTDA social service documentation policies and procedures.
  • Critical thinking, analytical, and problem-solving skills, as well as resourcefulness.
  • Demonstrated ability to be flexible and work independently as well as with a team.
  • Familiarity with housing vouchers (e.g., Section 8, FEPS, SEPS, LINC, FHEPS, SOTA, EOSD, City FHEPS).
  • Familiarity with SOI (Source of Income Discrimination) and Human Rights Law regulations.
  • Ability to assess needs and assist residents in developing housing plans for self-sufficiency and permanent housing.
  • Excellent analytical, evaluative, and human service management skills.
  • Strong organizational and leadership skills.
  • Supervisory experience with a team-oriented approach.
  • Must possess initiative and the ability to work well under pressure.
  • Ability to work both independently and as part of a team to complete complex projects in a fast-paced environment.
  • Must be adept and comfortable with crisis management.
  • Excellent oral and written communication skills coupled with strong interpersonal skills.
  • Ability to think and act calmly in emergency situations.
  • Attention to detail and problem-solving skills.
  • Excellent time management skills with the ability to prioritize assignments.
  • Proficient in MS Office, Outlook, Excel, PowerPoint, and Adobe.
  • May be required to work long hours and a varied schedule.
  • Ability to travel throughout various locations in the New York City area.
  • Ability to lift and move objects weighing up to 30 pounds several times a week.
