About the Role
We are looking for a Social Care Officer to further develop their skills in case work under the Care Act, including promoting independence and Direct Payments. Responsibilities include carrying out assessments of needs, reviews, working with customers and carers in the community, and building links with community agencies within the neighbourhood team of Adult Social Care. The ideal candidate will possess strong assessment and care planning skills and be passionate about delivering results.
