About the Role
Provide guidance, support, and therapeutic follow-up to enable clients to effectively utilize coping skills and resources throughout their time in the Partial Hospitalization Program (PHP) to successfully complete and transition into lower levels of care.
Responsibilities
- Ensure client adherence to program expectations and active engagement in PHP programming through case management and therapeutic support.
- Provide case management sessions, check-ins, and crisis support aligned with clients' Clinical Wellness Plan.
- Conduct case management sessions focusing on goal planning, developing sober routines, attending appointments, building sober networks, and creating after-care plans.
- Utilize knowledge of local clinical, medical, and community supports to help clients access resources for well-being, clinical wellness, and long-term recovery.
- Maintain up-to-date knowledge of available resources.
- Communicate weekly with family and community providers via meetings, phone calls, and emails as a direct treatment team member.
- Address clients' day-to-day needs using trauma-responsive interventions while maintaining a calm, welcoming, and professional demeanor.
- Prepare for new admissions by ensuring welcome materials are in place and rooms/beds are clean and orderly.
- Complete the admissions process, including greeting clients and families and completing necessary forms.
- Orient new clients to the facility, programs, and services.
- Immediately report any behavioral problems or safety concerns to management and counselors.
- Facilitate program groups and other activities as directed.
- Assist team members to ensure clients receive an unsurpassed level of service by supporting the recovery process.
- Manage the scheduling of PHP clients in collaboration with the treatment team and OPS Administrative Assistant.
- Help clients identify sponsorship opportunities.
- Arrange transportation for residents as needed for meetings and appointments.
- Counsel clients individually and in group sessions to achieve wellness plan goals.
- Monitor client condition to evaluate therapy success and adapt treatment as needed.
- Support the completion of FMLA, unemployment benefits, and/or disability paperwork.
- Refer clients to the appropriate level of care and assist clients and families during transitions.
- Facilitate crisis interventions when necessary.
- Assist clients in transitioning back to work, home, or school according to the continuing care plan.
- Function in a peer support role.
- Collaborate with clients' collateral supports to meet unique needs.
- Keep the treatment team informed of client updates and pertinent information.
- Prepare and maintain reports and case histories in Aura and Salesforce using proper documentation techniques.
- Collaborate with Continuing Care, CIT, Recovery Coaching, and Alumni departments for continuity of care.
- Develop, foster, and maintain relationships with alumni.
Requirements
- Associate or Bachelor's Degree in a healthcare-related field (e.g., Social Work, Psychology, DARC program).
- Active CASAC or higher credential.
- Minimum 2 years of Case Management experience supporting clients with co-occurring disorders.
