About the Role

The Senior Program Director will supervise the Health Home Care Management Program, including the Criminal Justice Health Home component. This role ensures high-quality, client-centered services through staff supervision and cross-departmental collaboration, aiming for measurable outcomes. The position also involves representing the program externally to support operational and financial performance.

Responsibilities

  • Develop internal and external relationships to drive program growth and enrollment.
  • Ensure administrative tasks, policies, and procedures are implemented and followed by program staff.
  • Review and update policies and procedures for compliance with regulations and best practices.
  • Monitor staff documentation for compliance with program policies.
  • Supervise Health Home Supervisors and program staff to ensure quality care management services.
  • Oversee Cabrini Case Management and grant spending.
  • Provide strategic direction for the Criminal Justice Department.
  • Ensure ongoing programmatic excellence.
  • Build long-term, cross-sector relationships to sustain or expand initiative impacts.
  • Lead, mentor, and cultivate a high-performing team.
  • Complete data reports for funding sources or internal departments.
  • Meet with clients and facilitate high-risk case conferences.
  • Identify and address staff knowledge gaps.
  • Oversee the efficient administration of the billing process.
  • Conduct monthly meetings with program directors to address operational challenges and implement solutions.
  • Collaborate with Data Operations and Compliance to enhance monthly reports.
  • Undertake special projects and other assigned duties.

Requirements

  • Bachelor’s degree in social work, human services, psychology, sociology, or a related field is preferred.
  • 3 years of experience in operating or supervising social services programs, preferably in care management.
  • Strong familiarity with care coordination/care management; CMAs and Health Home experience preferred.
  • Ability to handle sensitive issues with diplomacy and discretion.
  • Unwavering commitment to quality programs and project management to achieve strategic objectives.
  • Demonstrated integrity, positive attitude, mission-driven approach, and passion for the organization's mission.
  • Ability to work collaboratively with a management team.
  • Capacity to manage multiple tasks simultaneously in a complex environment.
  • Excellent interpersonal and communication skills (verbal and written).
