About the Role

The Prevention Family Specialist aims to preserve the family unit by providing supportive casework counseling, empowering parents with tools and resources for child safety and well-being, and advocating for quality services. This role is part of the Home Run program, a school-based initiative designed to prevent out-of-home placements for at-risk children facing school and family challenges.

Responsibilities

  • Conducts family strength and needs assessments, identifying problem areas and utilizing resources for growth.
  • Participates in the development, implementation, and evaluation of treatment and discharge plans.
  • Develops alternative placement or permanency resources for youth.
  • Completes comprehensive discharge summaries at least eight weeks prior to discharge.
  • Delivers case management services, including scheduling counseling sessions and ensuring medical/dental examinations.
  • Tracks services, completes documentation promptly and comprehensively, and ensures compliance with confidentiality standards.
  • Conducts casework contacts with families at least twice monthly in their homes and communities to focus on treatment goals.
  • Engages youth and families by building effective and caring relationships.
  • Ensures children are maintained in safe and secure environments.
  • Develops and maintains relationships with community schools and resource agencies.
  • Develops and maintains partnerships with courts, probation, and local law enforcement.

Requirements

  • Must have a minimum of 1 year of experience working with at-risk youth & families and case management.
  • Must be able to work a flexible schedule with variable hours, including day, evening, and on-call duties.
  • May require evening/weekend fieldwork.
  • Must have a Valid Driver’s License with an acceptable driving history and a reliable vehicle.
  • Ability to coordinate and plan travel efficiently for visits and appointments.
  • Ability to work with economically and culturally diverse populations.
