About the Role
The Case Planner is responsible for ensuring the safety, permanency and well-being for children placed into foster care. This role involves partnering with families, leading service planning activities, and facilitating the family’s plan through direct intervention and coordination with other providers.
Responsibilities
- Monitor safety and well-being through casework contacts and home visits.
- Co-develop, coordinate, and monitor the service plan in alignment with family needs.
- Ensure child's medical, mental health, academic, and housing/career goals are achieved in collaboration with the interdisciplinary team.
- Document all activities as progress notes in Connections within program-defined timeframes.
- Act as a key participant in the Family Team Conference and complete the FASP.
- Attend Family Court hearings and provide testimony on family progress.
- Complete court reports and Permanency Hearing reports.
- Ensure frequent, positive, and consistent contact between child and parents.
- Proactively problem-solve barriers towards reunification and other permanency options.
- Provide meaningful and responsive actions to families in times of need.
- Actively participate in treatment planning and Trauma System Therapy team meetings.
- Participate in job development and professional development activities.
Requirements
- Ability to perform essential job functions with or without reasonable accommodation.
- Ability to handle and resolve recurring problems.
- Bachelor’s degree in social work or related field required.
- Prior child welfare experience preferred.
- Driver’s license preferred.
- Ability to travel between Staten Island and Brooklyn locations.
- Work hours may vary based on program needs.
