About the Role
East Side House (ESH) is a community resource dedicated to expanding opportunities for individuals and families through education, career, and supportive services. The Social Worker will provide essential social work and supportive services to participants in Expectant Families and Older Adult Programs, helping them address immediate needs, enhance stability, and connect with vital resources.
Responsibilities
- Conduct initial and ongoing needs assessments and develop appropriate support, referral, and follow-up plans.
- Coordinate emergency assistance and basic-needs support, including documentation and follow-up.
- Provide short-term supportive counseling, crisis support, safety planning, and appropriate referrals.
- Connect participants and families to internal and community resources and public benefits.
- Plan and facilitate psychoeducational workshops, groups, and events.
- Collaborate with Home Visitors and other program staff to identify needs and coordinate services.
- Maintain relationships with community partners and connect participants to East Side House programs.
- Maintain timely, accurate, and confidential documentation of assessments, services, and outcomes.
- Fulfill mandated reporter responsibilities and respond to participant safety concerns.
- Participate in case consultation, program planning, and professional development activities.
Requirements
- Master of Social Work (MSW); social work licensure not required.
- Minimum of 2-3 years of relevant experience in social services, case management, family support, or community-based services.
- Experience working with expectant families, families with young children, older adults, and/or diverse community populations preferred.
- Demonstrated experience with needs assessment, resource navigation, referrals, advocacy, crisis support, and service coordination.
- Knowledge of New York City public benefits, emergency assistance, healthcare, behavioral health, housing, and food security resources.
- Experience developing or facilitating psychoeducational workshops or groups preferred.
- Knowledge of mandated reporting, participant safety, confidentiality, professional boundaries, and documentation requirements.
- Strong written and verbal communication skills.
- Proficiency with electronic case-management/data systems and Microsoft Office.
- Bilingual English/Spanish strongly preferred.
- SIFI Certification strongly preferred with at least one year’s experience supervising social work interns.
- Clearance to work at ESH, contingent upon successful completion of background checks and/or screenings.
