About the Role

The Service Coordinator works cooperatively with older adults to identify goals and locate support services that enhance their well-being and maintain housing within the Supportive Housing Program. This role is crucial for individuals aged 55+ who are homeless with active Medicaid, ensuring they receive necessary coordination services.

Responsibilities

Support and coordinate care needs with participants, their support systems, community providers, and medical professionals.
Complete Health and Wellness Assessments and care plans upon move-in and annually.
Utilize assessments and care plans to help participants achieve their goals.
Participate in food delivery and distribution to residents.
Facilitate support groups.
Participate in team meetings bi-weekly to ensure communication.
Participate in and potentially facilitate meetings between participants, supports, and service providers.
Document all encounters with program participants, providers, agencies, and supports.
Ensure participants have the necessary tools and support to maintain housing.
Facilitate referrals for indicated services, such as housing assistance, vocational rehabilitation, substance abuse treatment, day programs, and medical care.
Complete all required documentation for enrolled services within program timelines.
Assist participants in attending programs through verbal encouragement and in-person support.
Inform residents of local resources, including the ESLC Neighborhood Program, and support participants in these programs.
Cross-train and provide coverage for Medicaid Waiver Service Coordination Programs (TBI and Nursing Home Transition Diversion) as needed.
Attend weekly, monthly, and quarterly meetings.
Ensure participants maintain active Medicaid and assist with benefit management.

Requirements

Bachelor’s degree in Social Work or related field preferred.
Associate degree with 2 years of experience working with homeless individuals with co-occurring mental health, substance abuse, and physical health problems will qualify.
Valid New York State Driver's License required.
Reliable transportation.
Flexible, yet organized, with the ability to exercise independent, sound judgment.
Knowledgeable about community resources, Medicaid Services, and available non-Medicaid services.
Demonstrated ability to collaborate effectively in a team setting.
Ability to maintain effective and professional relationships with participants and the care team.
Strong communication skills.
Ability to effectively engage participants in a person-centered relationship.
Ability to meet with participants in their apartments and common areas.
Working knowledge of differential diagnosis of common mental health and/or substance use disorders, when appropriate.
Computer skills including working within a Windows OS and managing electronic documents and emails.
Ability to organize your day, work independently, and be flexible within a team environment.
Ability to perform physical requirements including walking, using stairs/elevator, standing for extended periods, occasionally lifting 30 pounds, sitting, talking, and hearing.
Specific vision abilities required include close vision, distance vision, peripheral vision, color vision, and ability to adjust focus.
