About the Role
The Crisis Nursery Family Resource Navigator will provide supportive case management services to families utilizing the program. They will conduct a needs assessment for families interested in receiving case management services and assist clients with referrals to community resources.
Responsibilities
- Crisis intervention and response
- Outreach, screening, intake and assessment
- Coordination, development, implementation, and ongoing assessment of family's case management needs, service, and plans
- Referral readiness, information, and facilitation
- Transporting program participants, as required
- Case management
Requirements
- Associate's Degree in Human Services or related field AND 1 year related experience OR equivalent
- A valid driver's license
- A telephone
- Background check
- Driving record and automobile insurance that meets agency insurance carrier requirements
Skills
- Strong problem-solving and decision-making skills
- Case management techniques and interventions
- Knowledge of community resources
- Knowledge of risk reduction and protective factors
- Knowledge of family service systems
- Strong literacy and communication skills
- Strong communication, time management, and multi-tasking skills
- Advanced knowledge of Microsoft Office
- Advanced knowledge of Internet Explorer
