About the Role

The Prevention Family Specialist aims to preserve the family unit by providing supportive casework counseling in the home. This role empowers parents with tools and resources to ensure children's safety, well-being, and permanency, while advocating for quality services.

Responsibilities

Facilitate the 10-session Independent Living Skills Curriculum a minimum of two times annually.
Teach life skills, financial literacy, employment readiness, communication, problem-solving, housing readiness, and community living skills.
Deliver the Everyday Parenting Curriculum to referred parents and caregivers.
Facilitate parenting education groups and individual parent education services.
Coordinate and facilitate up to two parenting curriculum groups annually.
Provide temporary case coverage for Family Specialists during planned and unplanned absences, vacancies, or leave periods for up to four cases.
Conduct required face-to-face contacts, family outreach, transportation services, wellness checks, and crisis response activities during coverage assignments.
Collaborate with Family Specialists, Program Coordinators, schools, county departments, probation, and community partners.
Complete all documentation associated with assigned and coverage cases within required timelines.
Attend and document Family Team Conferences, third-party reviews, and other meetings as required.
Complete, process, coordinate, and transport Respite youth as needed.
Transport youth and families to appointments, school meetings, court appointments, respite homes, community activities (summer/break programing), parenting programs, program Jump Start, and Independent Living classes.
Assist youth and families in developing positive connections within their communities.
Participate in the agency's Subject-to-Call rotation schedule.
Maintain availability during assigned on-call periods, including evenings, weekends, and holidays.
Maintain accurate and timely records within Evolv, Connections, and other required systems.
Complete audits on case files at required intervals.
Review documentation and provide administrative support as assigned by the Program Coordinator.
Coordinate annual holiday assistance and donation programs.
Coordinate back-to-school supply collection and distribution activities.
Monitor office organization, cleanliness, and professional appearance.
Track office supply inventory and submit replenishment requests.
Collect and distribute daily mail and packages.
Serve as Fire Safety Officer for the Schoharie Office, including completion of required drills and reports.
Assist with special projects and Prevention Services initiatives as assigned.

Requirements

Minimum Bachelor’s degree in human services or related field.
Minimum of 1 year of experience working with at-risk youth & families and case management.
Must be able to work a flexible schedule with variable hours including day, evening, weekend, holiday and subject to call hours.
Subject to call: Community Family Specialist is required to be on call in collaboration with a Program Coordinator.
May require evening/weekend field work.
On Call is in rotation with the program Family Specialists.
Must have a Valid Driver’s License with acceptable driving history to the Agency, and a reliable vehicle.
Travel is required.
Ability to coordinate and plan travel for visits and appointments in efficient manner.
Ability to work with economically and culturally diverse population.

Skills

Ability to handle crisis intervention in solution focused manner
Communication skills including written, verbal and listening skills.
Bilingual is a plus!
