About the Role
FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Jamaica, NY, to provide exceptional one-on-one care to older adults in their homes or senior living communities. This role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model, offering autonomy, flexibility, and meaningful patient outcomes. Clinicians can also incorporate telehealth and virtual care.
Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions supporting patient wellness, longevity, mobility, and functional independence.
- Document effectively within FOX’s EMR system.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth / virtual care when appropriate.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, providing moderate to maximum assistance with patient transfers, repositioning patients, standing/walking for extended periods, climbing stairs, performing therapeutic activities, and driving to patient homes.

Salary/Compensations
- Full-Time: $75,000 - $115,000 annually
- Part-Time and FOX Flex (PRN) will be pro-rated.
