About the Role
At Heart to Heart Hospice, we provide compassionate care to patients with life-limiting illnesses and their loved ones. We are dedicated to making a difference in the lives of our employees by fostering a supportive team environment and contributing positively to the communities we serve.
Responsibilities
- Provide home visits for patient assessment, treatment, and support, including pronouncement of patient death, crisis management, and admission.
- Plan comprehensive services for patients and families, developing or revising nursing care plans with physician and IDG input.
- Utilize excellent customer service skills with internal and external customers, including proper phone etiquette.
- Prepare clinical and progress notes demonstrating progress toward established goals.
- Inform physician and other personnel of patient needs and intervention outcomes, evaluating patient/family response to care.
- Submit all documentation in a timely manner.
- Perform other duties as required.
- Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
Requirements
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current State Registered Nurse License
- Current Driver's License
- Reliable transportation
- Ongoing valid and current auto liability insurance
