About the Role
Health Link Home Health and Hospice is seeking a compassionate, organized, and team-oriented Physical Therapist (PT) to join their team in San Francisco. This role offers the opportunity to collaborate with dedicated professionals and make a meaningful impact on patients' lives, supported by a strong back-office team.
Responsibilities
- Provide services within the scope of physical therapy, in accordance with the plan of care and in coordination with the Interdisciplinary Team.
- Follow the plan of care, provide treatment, evaluate patient's reaction to treatment, and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing.
- Maintain appropriate clinical records and/or notes and submit them according to company policy on company devices.
- Provide supervision to a PTA if assigned.
- Follow all company policies and procedures.
Requirements
- Current PT License to practice in California.
- Home Health experience: 1 year (Preferred).
- Current CA Driver's License.
- Ability to cover San Francisco and surrounding areas.
