About the Role
Apex Rehab Group is seeking an experienced, licensed Speech Therapist to join their team. This role involves assessing patient needs, creating personalized treatment plans, and documenting patient care to help patients reach their speech and wellness goals.
Responsibilities
- Provide therapy services to patients in their homes.
- Make evaluations and create treatment plans customized for each patient.
- Work with patients to set short and long-term goals for speech and wellness.
- Assist in the implementation of the treatment plan, adjusting as needed for successful outcomes.
- Maintain excellent records of patient needs, care, and outcomes.
- Work closely with patient families to ensure they have the necessary knowledge and tools for success.
Requirements
- Active NY state license for practicing speech therapy.
- Strong communication and interpersonal skills.
- Excellent organizational skills.
- Strong charting and documentation skills.
- Prior experience in home care is preferred but not required.
