About the Role
Responsible for implementing, monitoring, and on-going assessment of the housing and social service plan for all families assigned to the Essence Hotel Shelter.
Responsibilities
- Completes family intake, comprehensive family history and maintains accurate and timely family case records.
- Designs social service plan in cooperation with new residents.
- Secures entitlements for clients and serves as an advocate for families in case of conflict.
- Provides advocacy and assistance to clients in obtaining permanent housing.
- Makes referrals to special housing agencies.
- Meets with clients to develop strategies to meet their housing needs.
- Coordinates permanent housing search.
- Escorts families to view apartments, meet prospective landlords and real estate brokers, and completes housing applications.
- Makes referrals for additional services such as mental health and/or medical services.
- Serves as a liaison with daycare and schools to monitor admission and attendance of client’s children.
- Follows up with open child welfare cases, open court cases, and parole/police matters.
- Works collaboratively with Shift Supervisor and Resident Aides.
- Participates and attends the Interdisciplinary Team and Administrative meetings weekly.
- Completes and enters client case management records in the DHS-CARES system.
Requirements
- Requires an HS Diploma/GED and comparable experience.
- Previous experience with the homeless population is needed.
- Ability to establish workload priorities and balance diverse caseloads.
- Exceptional communication skills, both orally and in writing, with individuals and groups regarding complex and sensitive issues or regulations.
- Physical ability to perform the essential job functions is needed.
- Performs any duties as required to meet the needs of the program and the clients served.
Skills
- Bilingual Spanish/English is helpful.
